Full name of the candidate
Title/position of the candidate
Affiliation of the candidate
Homepage of the candidate
Area(s) of expertise of the candidate
Your motivation for the nomination (up to 200 words)

Nominations of multiple candidates by the same person are welcome. 
You do not need to check their availability at this stage but do indicate it if you already know they would be in the position to take on the role.
